Home Health Care Services
Certified Home Health Agency
The Public Health Nursing Division provides therapeutic and preventative services in homes and clinics throughout Orange County. Upon referral by the patient’s physician, home health services are provided to residents confined in their homes due to illness or disability. These services are particularly appropriate for individuals who need professional health care at home upon discharge from the hospital. Patients remain under their physician’s medical supervision.
Home health services include the following:
- Skilled Nursing providing a wide range of professional nursing skills including enterostomal therapy and wound management
- Physical Therapy
- Occupational Therapy
- Speech Therapy
- Registered Dietician
- Medical Social Work services assist the patient and family with social or financial problems which interfere with the medical well being of the patient
- Home Health Aide for personal patient care
- Evaluations for nursing home placements from the community (PRI & SCREEN)

Long Term Home Health Care
What is Long Term Home Health Care?
A coordinated plan of health care and social services delivered to disabled, infirm or invalid persons in their own home. It is an alternative of care for those who would otherwise require placement in a nursing home or health related facility. The program assists relatives or friends to care for persons who qualify for this program.
History
The Long Term Home Health Care Program was established by the NYS Legislature and is sponsored in Orange County by the Orange County Department of Health’s Home Health Agency and the Orange County Department of Social Services.
Choosing Long Term Home Health Care
The decision to care for an individual at home rather than in a hospital or nursing home is made by the professional health care team, the client and the client’s family. If the family is willing and able to provide the necessary care, if the home environment is conducive to and effective in maintaining or improving the client’s health and if the complex services of an institution are not needed, then home care may be the preferred plan of treatment.
Who is Eligible?
Persons who are seriously ill or incapacitated for a long period of time may qualify if the following criteria are met:

Financially eligible for Medicaid. (Private pay patients also accepted into the program)

Professional assessment has been made that nursing home level of care, either skilled or intermediate id required.

Physician approval.

Complete medical, nursing; social and environmental assessment indicated that this program is appropriate in meeting health needs in a home setting.

Long term home health care cost cannot exceed 75% of the average monthly rate of skilled or intermediate care facility in Orange County.

Ultimate approval for services is determined by the Department of Social Services.
