Adult Home Care

LONG TERM CARE POINT OF ENTRY SYSTEM

Orange County Office for the Aging, Orange County Department of  Health and the Orange County Department of Social Services have worked in collaboration with each other and with community based providers to implement Orange County NY Connects; the county's Long Term Care Point of Entry System. Staff from the Orange County Office For the Aging and  Department of  Social Services comprise the Point Of Entry, providing information and referral services to Orange County Residents who may need assistance with long term care needs, regardless of their age or payment source.

Information may also be obtained by contacting Orange County NY Connects Point of Entry Staff at (845) 615-3710 during the hours of 9:00 AM to 5:00 PM Monday through Friday.

THE HOME CARE UNIT receives referrals from Orange County NY Connects. This unit determines consumers’ appropriateness for the Medicaid funded Long Term Care Programs below:

The Personal Care Aide Program (PCA), provides Seniors and other Medicaid - eligible individuals with assistance with activities of daily living that are essential to maintaining health and safety. Services are provided through a licensed agency or county Consumer - Directed Personal Assistance Program Agency. The program also includes the Personal Emergency Response System.

The Consumer- Directed Personal Assistance Program (CDPAP), permits seniors and other residents greater freedom and flexibility in receiving personal care services. Under this program, the individual or responsible person acting on his or her behalf assumes full responsibility for hiring, training, and if need be, terminating the employment of the people providing the actual services.

The Long -Term Home Health Care Program (LTHHC) is a waiver program that offers a home care alternative to many of those who might otherwise require care in a long-term care facility. In addition to providing personal care aides, nursing, emergency response and various therapies, LTHHC enables qualified couples with income and resources above the ordinary Medicaid  eligibility level to continue to qualify for assistance and to remain at home.

The Managed Long Term Care Program provides health and long-term care services to adults with chronic illness or disabilities to better address their needs and to prevent or delay nursing home placement. Services include or but are not limited t nursing, physical therapy, occupational therapy, speech pathology, medical equipment and supplies, podiatry, dentistry, optometry, respiration therapy, transportation and social day care. This program is available to Medicaid recipients who are eligible for admission to nursing homes.

The Assisted Living Program is another safe alternative to residential placement. The program combines the residential services of an adult home with community long-term care. These may include personal care, room and board, housekeeping, nursing, physical, occupational and speech therapy, medical supplies and equipment, adult day care, home health care, and the case management services of a registered professional nurse.
