Community Health Outreach (CHO) is a county and grant-funded unit of the Health Department that provides public health education, intervention services, case management and prevention services to target high-risk communities and/or specific public health issues in Orange County. CHO provides education, information and referral to individuals who are at highest risk of contracting communicable and infectious disease and/or require medical management for acute or chronic health problems. The overall goal is to foster behavior changes that will result in positive outcomes through outreach, education, referral and case management with the following programs:
-ASTHMA EDUCATION -LEAD POISONING PREVENTION PROGRAM -COMMUNITY HEALTH WORKER PROGRAM -HEALTHY NEIGHBORHOODS PROGRAM -LYME DISEASE EDUCATION -MIGRANT HEALTH SERVICES -PUBLIC HEALTH EDUCATION -TOBACCO PREVENTION -WEST NILE VIRUS EDUCATION
